				Library Tour Request Form
				Library faculty will provide instructional sessions to first-time library users, introducing them to the library's online catalog and multi-disciplinary online databases. Instructional sessions are also provided to upper-level classes on applicable subject databases. Please be specific in outlining your research needs and submit requests as early as possible to avoid conflict.
				If Instructors wish to schedule a session for a class, they must submit the following Request Form or call the Bibliographic Instruction Librarian, at 482-6024 in advance of the preferred time slot. Consider alternative dates in case of scheduling conflicts. Instructors may also request a library literacy quiz to be distributed to their scheduled classes. The preferred method of scheduling a tour is by telephone.
				Complete the form below to request a library tour. The form will be emailed to the Bibliographic Instruction Librarian, who will contact the instructor by phone.
				Submission of the form does not indicate confirmation of scheduling.
				Overwrite each field with the pertinent information. All fields are required!
					Dr. Ms. Mrs. Mr.   Day of Week:   Mon Tue Wed Thu Fri Sat Sun Describe resources that need to be covered and/or class research assignments (for example, Literature Resource Center, MLA, Academic Search Premier databases).
